Quick-Start
Start from Apocalypse-like rules, flip the five realism switches, then change only one loot or population value per test world.
“Realistic” is author preference.
The game has no official Realistic preset. Choices like rare ammo, early utilities, and saliva-only transmission are playstyle recommendations mapped onto real 42.19 options.
Realistic Baseline
Fast Shamblers, Respawn None, Saliva Only, Water/Elec 14–30 days, Ammo Extremely Rare.
Best for a first realism run that still lets you build a base.
Best for groups that want realism without a grind wall.

2) Zombie Respawn

3) Transmission

Core switches
Labels match Build 42.19 Custom Sandbox. Tweak one value, then rerun the test loop in a fresh area.
There is no ordinary “0–14 days” slider. Instant and 14–30 Days are the early menu options. Exact day pins are advanced shutoff modifiers, not the primary Custom Sandbox control.
Zombies & pacing
Zombie Lore options plus Advanced Zombie population multipliers.

Generated output
Human checklist plus a server settings fragment. Paste into notes, or merge into your server sandbox file after a full backup.
15-minute test loop
Use the same loop every time in a fresh area so you tune feel, not guesswork.

Checklist
Stop when stable. Change one setting and rerun.
What you are measuring
Can you plausibly eat for 48 hours without a mega-mall?
Do basic life tools (bag, hammer, saw) appear within a day or two?
Do zombies migrate back, not magically respawn behind you?
Does one bad swing cascade into fatigue, wounds, and noise?

Each button applies one conservative tweak to the builder using Build 42.19 options.

What “realistic” means here
Realistic is a triangle: plausible consequences, still playable recovery, and repeatable lessons after death. It is not “maximum misery” and not an official game mode.

Some official presets sit between labels. This builder sticks to menu labels so you can rebuild the same world from the new-game screen.
Utility shutoff options
Custom Sandbox offers Instant, several 14-day-start ranges, multi-month ranges, and Disabled. Exact day pins are advanced shutoff modifiers, not the main menu control most players use.
How to apply
New game → Custom Sandbox, start from Apocalypse if you want a known baseline.
Set the five switches first (Multi Hit, Respawn, Transmission, Utilities, Ammo).
Set Rear Vulnerability to High, Medium, or Low — not an on/off toggle.
Only then tune population start/peak mults and peak day.
For dedicated servers, stop the server, back up the world and sandbox settings file, merge carefully, then restart. Transmission changes do not roll back bites already in progress.
